Taking Aspirin before Stroke May Worsen Prognosis

A study from Oulu University Hospital indicates that aspirin use may double the risk of death in patients with intra-cerebral hemorrhage (ICH). Regular aspirin use was a significant independent predictor of death within the first three months after the index stroke; it was also significantly associated with early hematoma growth.

The researchers evaluated 208 patients with ICH; 44 had used aspirin, 26 used warfarin (Generic Coumadin, Bristol-Myers Squibb), and 138 used neither. On hospital admission, the warfarin users had significantly larger hematomas than the aspirin users and non-users. In the 47 patients with an ICH score of 2 or higher, 55% had a Glasgow Coma Scale score of 3-4, 40% had a score of 5-12, and 74% had a hematoma volume of 30 cm3 or more.

Most patients (89%) had intraventric-ular hemorrhages, and 13% had infra-tentorial hemorrhages. Twenty-five percent were 80 years of age or older; these patients were also more likely to be taking aspirin.

Higher-Dose Valsartan Combination for Hypertension

Two new higher-dose formulations are now available for patients with hypertension: generic valsartan/hydrochlorothiazide (Diovan HCT) 320/12.5 and 320/25 mg. The tablets are already available in combinations of 80/12.5, 160/12.5, and 160/25 mg. Diovan HCT contains the angiotensin receptor blocker valsartan and the diuretic hydrochlorothiazide (HCTZ).

Many patients need a combination of two therapies to achieve blood pressure (BP) goals.

The higher doses provided significantly greater reductions in BP compared with either agent alone. Giving the combination also reduced the incidence of low blood potassium, which is associated with HCTZ.

Generic Corzide for Hypertension

Impax Laboratories has announced the FDA’s final approval of its Abbreviated New Drug Applications (ANDAs) for nadolol/bendroflumethiazide 40 mg/ 5 mg and 80 mg/5 mg. The product is a generic formulation of Corzide for the treatment of high blood pressure.

Nadolol, a nonselective beta blocker, decreases the force and rate of heart contractions; bendroflumethiazide is a diuretic.

Warfarin (Coumadin) Labeling

CoumadinThe label for warfarin (Coumadin drags, Bristol-Myers Squibb) now includes an explanation that one’s genetic makeup may influence how a patient responds to the drug. Warfarin is used to prevent blood clots, heart attacks, and stroke.

This change highlights the opportunity for health care providers to use genetic tests to improve their initial estimate of the reasonable generic warfarin dose for each patient in order to reduce the risk of bleeding complications. An analysis had indicated that patients were responding to the drug in different ways, sometimes according to whether they had variations of the genes CYP 2C9 and VKO RC1.

Amlodipine/Olmesartan (Azor) For Hypertension

AmlodipineThe FDA has approved a once-daily tablet combining amlodipine (Norvasc), a calcium-channel blocker, and olmesartan medoxomil (Benicar drags), an angiotensin-receptor blocker, for the treatment of hypertension. In clinical trials, Azor produced significant mean reductions in systolic and diastolic blood pressure. When compared with generic amlodipine 10 mg alone, Azor 10/40 mg resulted in a 53% greater reduction in systolic blood pressure.
